Memory Care

At Hearthstone at Murrayhill we celebrate the individual. When your loved one needs Memory Care, we work with everyone involved — the new resident, family members and staff strive to arrive at a detailed plan for the care and services best suited to each resident's needs. Our monthly rate includes all levels of cares * — which may reduce the family's anxiety about their loved one 's increasing care needs .

Individual Care Plans are reviewed regularly and updated as needed by Hearthstone's Health Services team. Caregivers are assigned to individual residents and are trained to provide care following the "Best Friends Model." Special attention is given to each resident to optimize their nutritional, physical and social needs.

Prior to moving in, each resident and their respective family members meet with one of Hearthstone's nurses to determine the resident's personal care needs. Every three months, or more often as needed, one of our nurses will meet again with the family to discuss the resident's changing needs and update the resident's Individual Care Plan.

Memory Care Services

Full-time Nurse
Specially trained staff on-site 24 hours a day to assist with all aspects of care while maximizing and encouraging the resident's independence
Three nutritious meals a day, including special diets and snacks
All utilities, including cable television
Secured interior and exterior doors and courtyards
Emergency response system
Beauty shop services available

Monthly "Share Group" meetings for family members and friends
Coordination of transportation needs
Fully supervised activities, social programs and outings
Weekly housekeeping and laundry (both linens and personal)
Use of all Hearthstone common areas when accompanied by family or staff including billiard room, putting green, movie theater chapel, exercise room, garden courtyards and library

* Occasionally an extreme situation may require assessment of a surcharge. Hearthstone's Health Services team will notify the family in these situations. Please note that monthly rates are subject to annual rent increases.
